notarized consent for tattoing & piercing minors
Minors in Florida must have notarized written consent from a parent or legal guardian before patronizing a body piercer or tattoo artist. Completing the Florida Notarized Minor Consent Form prior to meeting with me will help to streamline the process.
Body Piercing
In the state of Florida, a person may not perform body piercing on a minor without the written notarized consent of the minor’s parent or legal guardian.
An establishment may not perform body piercing on a minor under the age of 16, unless the minor is also accompanied by a parent or legal guardian, as specified in section 381.0075, F.S.
Non-compliance constitutes a misdemeanor of the second degree (section 775.082 or section 775.083, F.S.)
Tattoos
In the state of Florida, a person may not tattoo the body of a minor child who is at least 16 years of age, but younger than 18 years of age, unless:
(a) The minor child and his or her parent or legal guardian each submit proof of his or her identity by producing a government-issued photo identification
(b) The parent or legal guardian submits his or her written notarized consent in the format prescribed by the department
